DSS Sick Notes
For the first seven days a patient can “Self-Certificate” – you can pick up one of these forms from our reception. After the first seven days patients will need to book an appointment with the doctor to obtain another sick note. Sick notes can be back-dated and renewed in certain circumstances if needed.
You can get your NHS Prescription for free, providing you are:
Under 19 in full time education,
Aged over 60,
A War Pensioner
On Income Support or Family Credit,
Pregnant or with children aged under one,
Suffering with a specific medical condition such as epilepsy, thyroid and Addison’s disease, stomas, myasthenia or some types of diabetes.
A Cancer patient receiving treatment.
