Providing you are within the boundaries as indicated on the Practice Area map, we are happy to register you at our practice. You can specify at that time the doctor you would wish to see, although there may be the necessity to refuse your request because of availability. However, you will be able to see any doctor within the practice, providing they are holding a non-duty surgery on that day and they have appointments available.
In order to Register with the Homewell Practice, we ask all patients to complete a GMS1 Form, this form will have to be printed off and handwritten, or picked up from our surgery and completed.
If you are aged 16 or above please complete Registration Form A in addition to the GMS1.
For 15 and under please complete Registration Form B, in addition to the GMS1.
In addition to the GMS1 and the Registration Form, we ask would all patients please complete an Alcohol Questionnare and return it to our Reception Team.
Please print the necessary forms and deliver to the practice. This helps gather any information we need from your last doctor, including family history.
In accordance with Government guidelines, all new patients will be asked to produce photographic identification and proof of residence when registering at the Practice, in order to help reduce fraudulent registrations. Please bring with you one from each of the groups below:
GROUP 1 – PHOTO ID – ORIGINALS ONLY

GROUP 2 – PROOF OF RESIDENCE – CAN BE A PHOTOCOPY

Paid Utility Bill
Credit Card Statement
