The surgery offers a secure online system that is integrated with our practice appointment and prescriptions system.
In order for you use the system for booking appointments or reguesting repeat prescriptions, you must first register by completing the application form. This can be printed from the link below, along with a copy of the practice guidelines.
The service is free to our registered patients over 16 years of age and is available day and night, up to 365 days a year.
Requesting your Patient Access account details
If you wish to use Patient Access we ask that you read a copy of our practice guidelines. You must then complete and sign an application form. Both documents are available from reception or you can download them by clicking the links below.
Return your completed form to reception or post it to the surgery. We will then set up your account and create a letter containing all your account details.
For security reasons you must collect this letter from the practice in person as we cannot give out account details by phone, post or email. If you have poor mobility and are unable to get to the surgery, you may nominate a relative or carer to collect the details of your behalf (at your own risk) by writing their details on the application form.
Please allow at least five working days for us to set up your account before coming to collect your letter, and bring some photo identification with you (such as a passport, student card or driving licence) as we will need it to verify your identity.
