The Homewell Practice has set up a Patient Participation Group that is “virtual” in that most of the discussions take place via email and/or our Facebook Group.
We would like to work with our patients to ensure the services we offer truly meet the needs of all our patients.
If you would like to be involved with our new and exciting patient group, please print and complete the Patient Reference Group Contact Form, giving us your email address and up to date mobile number. Bring the completed form in to Reception, place in the “secure box” or give to a PRG representative.

Each year the group identify three areas they feel could benefit from attention where improvements or enhancements could be made.

Care Navigator Pilot – Dementia Support
This is a project aimed at supporting patients suffering from dementia and those caring for them. The Group’s aim is to set up a regular weekly session at the Health Centre where patients referred to the group can come along and discuss their needs in confidence so that they can be supported. The group would also like to work towards a directory of dementia-friendly establishments and community services. We will publish more information on this page as the project continues.
There will be 4 meetings held in the area in May, hosted by Portsmouth NHS Trust, to involve the public in the planning of care for patients with dementia. Please click on the link for more information.
